Assumption of Risk
I hereby acknowledge that I am voluntarily participating in personal training sessions conducted by Stoic Bodywork LLC at 589 Bethlehem Pike, Suite 500, Montgomeryville, PA 18936.
I understand that personal training involves inherent risks of injury, including but not limited to sprains, strains, fractures, and other physical ailments, as well as cardiovascular and heart health risks including cardiac events. These risks may arise from the use of exercise equipment, physical exertion involved, and the potential for accidents.
Waiver and Release of Liability
I hereby assume full responsibility for any and all risks associated with my participation in personal training sessions, whether such risks are known or unknown to me.
I agree to release, discharge, and hold harmless Stoic Bodywork LLC, its employees, staff, agents, and insurers from any and all claims, demands, damages, liabilities, or causes of action arising out of or in connection with my participation in personal training sessions.
Important: It is strongly recommended that you consult with your physician before beginning any new exercise program. By participating, you represent that you are physically capable of participating in personal training activities and have no medical conditions that would prevent you from doing so.

Therapeutic Bodywork
Consent and waiver terms for Therapeutic Bodywork, including Fascia Reprogramming and the unadvertised Muscle Sculpting service.
Cancellation — Therapeutic Bodywork
A 24-hour notice is required for cancellation. Cancellations within 24 hours will incur a $25 fee. This fee is waived once every 6-month period starting from your first scheduled appointment.
Consent to Bodywork
If I experience any pain or discomfort during a session, I will immediately inform the practitioner so that the work may be adjusted to my level of comfort. I understand that participation is voluntary and that I may ask to modify or stop the session at any time.
I understand that Therapeutic Bodywork is not a substitute for medical examination, diagnosis, or treatment. I should see a physician, chiropractor, or other qualified medical specialist for any mental or physical ailment of which I am aware.
I understand that practitioners at Stoic Bodywork do not perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness, and that nothing said during a session should be construed as medical advice or diagnosis.
Because bodywork should not be performed under certain medical conditions, I affirm that I have disclosed all known medical conditions and answered all intake questions honestly. I agree to keep the practitioner updated about changes in my medical profile.
Bodywork & Postural Assessment Waiver
I understand that Therapeutic Bodywork and related postural assessments may involve touch, pressure, movement, and physical evaluation that could cause temporary discomfort or aggravate an existing condition. I accept the risks associated with voluntary participation and agree to communicate any discomfort, limitation, or change in my condition.
I certify that I have no known medical condition that prevents my participation. If I have concerns about whether bodywork is appropriate for me, I will consult a qualified healthcare provider before participating.
I understand that the practitioner may modify or terminate a session at any time when they believe doing so is necessary for safety or well-being.
I assume full responsibility for the ordinary risks associated with my voluntary participation. To the fullest extent permitted by law, I release and hold harmless Stoic Bodywork LLC, its members, employees, practitioners, agents, representatives, and contractors from claims or liabilities arising from my participation in Therapeutic Bodywork or a related postural assessment, including claims arising from ordinary negligence. This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that cannot lawfully be waived.
Additional Consent — Muscle Sculpting
I understand that Muscle Sculpting is a manual manipulation modality that works on tissues that may become sensitive during work. This may become excessively painful at times and I will inform my practitioner when I need a break. I further understand that there may be swelling, bruising, and other temporary side effects from the treatment. I assure the practitioner I will stop the treatment at any time that I do not feel it is progressing towards my goals, whether personal or competition related.

Holistic Alignment
Consent and participation terms for in-person Energy Sessions and in-person or virtual Coaching Sessions.
Cancellation & Payment
A minimum of 24 hours' notice is required to cancel or reschedule an Energy Session or Coaching Session. Whether a charge applies to a late cancellation is determined by the practitioner based on the circumstances.
Virtual Coaching Sessions require payment or a card on file when booking. Payment for a virtual session cancelled with less than 24 hours' notice may be forfeited. At Andrea's discretion, a partial credit may instead be carried forward and applied to the client's next booking.
Consent to Energy Sessions
I understand that Energy Sessions are voluntary, non-medical wellness services intended to support relaxation, stress reduction, and body awareness. They do not include medical or mental-health diagnosis or treatment and are not a substitute for care from a qualified healthcare professional.
Energy Sessions include chakra balancing and may include light touch. Before an in-person session, I may consent to light touch, decline light touch, or ask Andrea to request permission each time. I may change or withdraw that consent at any point during the session.
Sessions ordinarily take place on a massage table. If a massage table is not viable for me, I may request that the session be completed in a chair.
Meditation, intention setting, aromatherapy, or sound healing may be incorporated based on my preferences and the needs discussed for the session. I agree to tell Andrea about sensitivities, allergies, health concerns, or accommodations that may affect my participation.
I understand that individual experiences vary and no particular outcome is promised or guaranteed. I may ask to modify or stop any practice at any time.
Consent to Coaching Sessions
I understand that Coaching Sessions are collaborative wellness and personal-development services that may use somatic techniques, guided visualization, meditation, energy practices, reflection, and practical exercises focused on awareness, patterns, values, and personal goals.
Coaching is not psychotherapy, professional counseling, medical care, diagnosis, or treatment. Andrea does not provide emergency or crisis services through Coaching Sessions. If my needs fall outside the scope of coaching, I understand that I may be encouraged to seek support from an appropriately qualified professional.
I remain responsible for my own choices, actions, and well-being during and between sessions. Reflection or somatic practices may bring up discomfort or difficult emotions; I may decline, modify, pause, or stop any practice at any time.
I understand that individual experiences vary and no particular outcome is promised or guaranteed.
Additional Consent — Virtual Coaching
Virtual Coaching Sessions are conducted through Google Meet. I am responsible for joining from a setting that is physically safe, reasonably private, and appropriate for reflection and movement.
I understand that electronic communication may be interrupted by connectivity, equipment, or platform issues. Although reasonable care will be used, privacy cannot be guaranteed beyond the security and privacy controls available through the selected platform and my own device and environment.
If a connection is interrupted, Andrea and I will make a reasonable effort to reconnect. If the interruption prevents the session from being completed, Andrea may continue the remaining time on another platform, reschedule the remaining time, or issue a partial credit toward a future booking.
Voluntary Participation & Assumption of Risk
I voluntarily choose to participate in Energy or Coaching Sessions and understand the nature and intended scope of the service I selected. I agree to communicate relevant concerns, limitations, and preferences and to stop participation if I believe continuing is not appropriate for me.
I understand that these sessions may involve light touch, seated or reclined positioning, gentle movement, meditation, visualization, somatic attention, or emotionally reflective conversation depending on the service and my consent. Possible responses may include temporary physical or emotional discomfort, fatigue, or heightened awareness.
I assume responsibility for the ordinary risks associated with my voluntary participation. To the fullest extent permitted by law, I release and hold harmless Andrea Hackman, Stoic Bodywork LLC, and their respective members, employees, agents, representatives, and contractors from claims or liabilities arising from my participation in an Energy Session or Coaching Session, including claims arising from ordinary negligence. This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that cannot lawfully be waived.
Adult Clients & Practitioner Relationship
Andrea provides Energy Sessions and Coaching Sessions only to clients who are 18 years of age or older.
I understand that Andrea may provide services as an independent contractor operating through or at Stoic Bodywork LLC. Stoic Bodywork LLC does not control the professional methods Andrea uses within the agreed scope of an Energy Session or Coaching Session. These consent, conduct, scheduling, and safety expectations apply regardless of that business relationship.
